DESCRIPTION:\nEllen Koski, MPH, CPH is the Executive Director of the Fox Valley Advance Care Planning Partnership, a collaboration between ThedaCare, Ascension NE Wisconsin, Mosaic Family Health, and the Fox Valley community to make advance care planning and conversations an integral part of living. Ellen is a community health educator by training and seeks to identify and impact knowledge, skills, and environments necessary to support healthy communities.\n \nBen Adams joined McCarty Law LLP in Appleton in August of 2019 after practicing with Adams & Woodrow, S.C. in Neenah for over 30 years.  He continues to emphasize elder law, special needs planning, guardianships and alternatives to guardianship, nursing home, and long term care issues, advance care planning for health care decision making, and estate planning in his practice. Ben has been actively involved in the Fox Cities Coalition for Advance Care Planning since its inception in 1999 and is a member of the community leadership team of the Fox Valley Advance Care Planning Partnership, created in 2016.\n \nJon Fischer joined McCarty Law after finishing his undergraduate and law degrees at the University of Wisconsin. His practice focuses on three main areas as he aims to help clients in their time of need. These include business formations and transactions, commercial and residential real estate matters, and estate, probate and Medicaid planning.\n
